Olympic Physicians has joined a Collaborative Team in partnership with the Washington State Department of Health to study the Medical Home.  The study will serve as a model as the nation looks towards health reform.

Some of our initial efforts are focusing on projects that help patients gain access to their Olympic Physicians provider upon hospital discharge, improving the phone system, and redesigning internal processes. 

We are excited as we embark on this two year project that will hopefully lead to Medical Home Certification.
